AUTHORIZATION FORM

(to cover any activities outside

the school premises

Dear Parent/Guardian,

During the academic year the school organizes a variety of trips and excursions.

These trips will always be supervised by staff from the school.

This form gives your child/ren permission to leave the school grounds, for whatever reason.

The school cannot be responsible for the acts of third parties, or events which occur on an excursion which are not the responsibility of the teacher in charge of the excursion:  for example - to put it at its worst - the coach could be involved in an accident which was caused by the driver or drivers concerned, people who are not directly employed by the school.

It is the responsibility which the school and its staff cannot take.  This places the responsibility on the party or parties causing the incident.

To partake in official school excursions.  I/we hereby relieve the school and its staff from any responsibility for occurrences which do NOT pertain to the guarantee of security and vigilance shown by the school and its staff.  PROPER NOTICE AND SUPERVISION WILL BE PROVIDED.
